How to Use these Protected Data Forms

Using the secure forms allows one to type in information in the fields provided without changing the form itself. 

  • Double click the file you want to download. 

  • Choose "Save" when prompted to either save or open your document. 

  • Save the document to your Desktop. 

  • Double click your document on the desktop to open, and when prompted to "Enable Macros,"  please choose yes.  

  • Enabling macros will allow the Spell Check feature to run when you close and save your document.

  • Each field will expand as you enter information. A shaded box indicates a field.

  • Increasing the number of fields, rows or text space is not possible.

 

Entering information on the form:

  • Use the TAB key to move forward from field to field.

  • Use the SHIFT-TAB keys together to move backward from field to field.

  • Use the SPACE BAR by tapping it once to mark a box; tap a second time to remove the mark in the box. Without the cursor in the box, clicking the left bar on the mouse will also place and remove a mark in a box.

  • For a drop-down box, click on the field or down arrow and highlight and click on your selection. The selected information (e.g., disability classification) will appear.

  • Use the SCROLL BAR to move through the document, as it will not automatically scroll down. Also use the SCROLL BAR to skip over a sections both forward and backward.
